Hello DJchot!
I check mo ang book na legal forms written by Guevarra, authority yon sa pagsulat ng mga legal documents. Since an SPA needs to be notarized kelangan mo itong dalhin sa Philippine Embassy para maging binding. If you have more questions just send me a PM.

nacarnap kasi yung car ng sister ko. under my name pa yon kaya di nya malakad yung claims nya sa insurance.
the last time na nabangga sya, i was in thailand noon at kinailangan nya ng SPA rin para maka claim sya sa insurance. nagpagawa ako ng SPA sa phil embassy nga sa thailand. di kasi tinanggap yung blank SPA na pinirmahan ko dahil dapat daw, notarized sa kung saang bansa ako nandoon.
ganon din kaya uli?

Oo DJChot, kelangan i-execute mo yon sa bansa kung saan ka andon at ipa notarize mo. Di pwede ang blank SPA. Since it’s called special power of attorney kailangan specific ang purposes mo and it should be clearly embodied in the document. Kung GPA (General Power Of Attorney) okay lang kahit blanko kasi nga general as opposed to special na kailangan specific.
I am sure marami dito sa TF na alam kung saan ang embassy. ANd about the form, nakakita ka ba? Sa title na Legal Forms mo to mahahanap. Kung ala kay guevara, try the ff authors: Pineda, Pena. Sorry I forgot their first names.
